A learning story may be linked to a plan from within the story to either a planned activity which the story relates to or to a follow up plan which was inspired by te observations made and documented in teh story.  Once a learning story is linked to a plan you can see the linked documents in the threads display which can be viewed in any learning story at the bottom of the page.  Take a look at this video to see how easy it is to link stories to plans.
